Book Changes


If you desire to change a textbook used in your class, you must notify the University librarian by the end of week 6 of the quarter prior to the requested change providing the librarian with the information on publisher, author, title, ISBN number, edition, etc.  Requests for changes in textbooks must be accompanied by the Textbook Requisition Form which is available in the library or on CANVAS.  Please note, if you are co-teaching this class (theory) with other instructors, you must both agree on the change and sign on the form.  All major textbook changes must have rationale using the ARAP Form as to the need for change discussed in the CURRICULUM COMMITTEE first then General Faculty meeting.  MAJOR substantive change on TEXT BOOK MUST BE APPROVED BY THE APPROPRIATE DEAN AND PROVOST.  Once approved, the Syllabi will need to be updated.  IF the textbook change is ONLY for an updated edition, ONLY signature of the Dean is required.
