Clinical Evaluation

Clinical evaluations are composed of the following documents for which faculty and students are required to complete at the end of each clinical rotation:

1. Student Clinical Facility Evaluation
2. Student Clinical Faculty Evaluation
3. Faculty Clinical Student Evaluation
4. Faculty Clinical Student Mastery Level Evaluation
5. Clinical Facility Evaluation of Students and Faculty. Faculty assigned to the facility must ensure completion of above clinical evaluations.

All of the above evaluations are available in VIA and EvaluationKit. Please seek assistance from the Office of Compliance and Quality Assurance at compliance@auhs.edu.
